Payment is due at the time of service unless other arrangements have been made in advance. As is typical practice in the Carmel and north Indianapolis area payments for mental health services are made directly to the provider. It is the responsibility of the patient and/or patient’s family to pay for services rendered. It will be the patient's responsibility to contact their insurance company before, and during the treatment process, to determine whether or not services will be covered by the insurance company. Also, the patient will need to obtain from the insurance company the necessary forms for documentation for the coverage of services. The patient will need to obtain and file all necessary forms to obtain reimbursement from the insurance company. This office will cooperate with the patient's efforts to provide necessary documentation to insurance companies. Acceptable payment for services is a personal check.

Managed Care Free Practice:

This practice is managed care free. As such, and is common in this geographical area for mental health services, this provider will not participate on managed care panels.

Fees:

Fees are consistent with area norms for similar services. The details of such fees can be discussed over the phone or in person with this office.
